Consultations
New client consultations are free of charge. A Manager of Care will meet with your family to determine your needs and answer any questions you may have. The Manager of care will also explain our mission and services and fill out any admission or client intake forms that are necessary.
Initial Nursing Assessment
Initial nursing assessments are performed by our Registered Nurse. The RN will perform a full evaluation of multiple systems, vitals, review medications and past medical history, discuss any needs with you and your family and consult with appropriate medical personnel such as the primary care physician, specialists, case workers and other health care professionals that may be involved. Once the assessment is complete the Registered Nurse will recommend the appropriate services to be provided and at what interval the recommended services may be needed.
Develop a Personal Plan
Our Registered Nurse will create an individualized Plan of Care, and then will supervise all caregivers/aids that are assigned. This assures the best and most cohesive care possible.
Re-Evaluations
Our Registered Nurse performs updates on a quarterly basis in order to keep the Plan of Care up-to-date. This is done so that Luba’s caregivers can provide the most comprehensive care and ensure our clients stay healthy, independent, and safe.
